Module 1: Introduction (00:00:00 - 00:04:39)
Mr Ankur Desai from Ethicon introduced and welcomed the surgeon for the day, Dr Devendra G. Parikh, Robotic & Minimal invasive GI & Thoracic Cancer Surgeon, the moderators for the day Dr Jagdish Kothari, Dr Dhaval Rajde and Dr Abhishek Jain, and the participants. The poll was raised and the structure of the webinar and the general instructions were shared with the participants before handing over the stage to Dr Devendra G. Parikh.
Module 2: Surgery (00:04:39 - 01:50:19)
Dr Devendra Parikh started the surgery by first giving a brief patient history. Dr Parikh introduced his team, explained the patient and the port position in detail. Dr Devendra Parikh walked the participants through the steps of the procedure to be performed in the surgery. The settings were shared. Doctors view of performing surgery upfront or after NACRT was shared during the surgery. The benefits of using Harmonic while handling the lymph nodes were shared. Dr Devendra also shared which lymphadenectomy he prefers. What should be done when the surgeon wants to preserve the thoracic duct and what is done when you want to resect it was shared. Anaesthetists were introduced and thanked. The vitals maintained throughout the surgery were also shared. Time is taken to heal the approximated ends of the oesophagus after anastomosis was shared. Dr Devendra Parikh explains the diet after the surgery which should be followed by the patient.
Many questions were raised during the session which was moderated by Dr Jagdish Kothari, Dr Dhaval Rajde and Dr Abhishek Jain which were answered by Dr Devendra G Parikh.
Module 3: Presentation on Megadyne Smoke Evacuator (01:50:19 - 02:15:47)
The video was played introducing megadyne smoke evacuator. Its advantages and uses were shared in the video.
Module 4: Surgery (Abdominal and Neck part) (02:15:47 - 04:00:59)
After the patient was prepared for the abdominal and neck part, Dr Devendra Parikh started the surgery for the same. All the port positions were explained. The dissection was done and shown in detail.
Stations that will be addressed were shared by Dr Devendra Parikh. The reason why hiatal dissection is usually kept for the last was shared. Post the abdominal part, the surgery for the neck region was performed.
The type of incision made for neck incision was explained. The dissection between the oesophagus and pre-vertebral fascia was shown. The oesophagus was cut keeping the length enough to do the anastomosis.
The importance of doing feeding jejunostomy was shared by Dr Parikh. The anastomosis was shown. The neck wound was closed after giving a thorough wash.
Many questions were raised during the session which was moderated by Dr Jagdish Kothari, Dr Dhaval Rajde and Dr Abhishek Jain which were answered by Dr Devendra G Parikh.
